By attaching equal importance to hidden near-miss risks and formal accidents, the enterprise can realize risk interception in advance, curb the evolution of potential hazards into major abnormal events, and form a forward-looking risk prevention mechanism complementary to accident post-disposal management. ## 1. Near-Miss Risk Scope and Identification Standards Typical anti-corrosion near-miss scenarios include slight pipeline seepage that is discovered before leakage, temporary parameter drift that recovers automatically, incomplete cleaning operation corrected before production, overdue instrument calibration found in advance, improper spare parts preparation replaced before installation, irregular temporary wiring easily causing damp corrosion, and misoperation behaviors stopped in time. Any unsafe state, non-standard operation, environmental hidden trouble or management defect that has not led to loss shall be classified as a near-miss event. Concealment, delayed reporting or downplaying of near-miss risks is deemed a serious violation of internal management rules. ## 2. Anonymous reporting channels are opened to encourage frontline staff to actively submit hidden risks, and reasonable reporting behaviors are protected from accountability in accordance with Doc.106 incentive provisions. ## 3. Graded Risk Analysis and Root Cause Investigation Near-miss incidents are graded by potential hazard severity corresponding to the blue-yellow-red three-level risk framework. Blue-level minor near-miss risks are analyzed by the workshop team to summarize operational standardization loopholes. Yellow-level potential equipment damage risks require cross-post joint analysis to explore defects in patrol mechanisms and training arrangements. Red-level near-miss events that may induce major corrosion leakage trigger multi-department root cause analysis, checking links including material quality, process setting, responsibility division and system clauses. All analysis results must clarify direct, indirect and fundamental causes rather than simply attributing risks to personnel carelessness. ## 4. Regular Statistical Summary and Warning Popularization The equipment department sorts out all near-miss records monthly, summarizes high-frequency risk types and concentrated occurrence areas, and compiles typical near-miss cases into the enterprise anti-corrosion knowledge base for regular warning training. Near-miss statistical data is taken as an important reference indicator in the annual system review to optimize anti-corrosion process parameters, on-site management standards and post competency assessment content.
